Billing Instructions 
Most of the billing instructions are available for download in a portable
document format (PDF).  In order to read and print these billing
instructions, you must have the Adobe Acrobat Reader.

We recommend that you use version 4.0 in order to take full advantage
of the features offered in some of the files. Earlier versions may not
