Palliative Care Team/Program

Description:
Delivers individualized palliative care to clients at their home/institution utilizing a team of nursing and home support staff specially trained in theoretical and practical palliative care skills. This program training includes: communication, pain symptom control, personal care, dying process, grief process and stress management.

Individuals who are facing an illness that is no longer responding to curative care.
