Description:
In-Home Client Services provides coordinated delivery of appropriate multi-disciplinary health care services to the client, in the home or school, when this is in the best interest of the client and within the limits of the available program resources. These services are directed at helping the client attain and maintain the optimum achievable level of rehabilitation to independent functioning.

Eligibility:
For clients who can be adequately treated at home/school, whose need cannot be met on an outpatient basis, and whose home or school is suitable to enable the provision of care. Client's family will participate when required.
