Description:
Provide assistance to ambulatory patients who require transportation from their home to and/or from cancer treatment for surgery, radiotherapy, chemotherapy or a follow-up appointment.

Individuals diagnosed with cancer (Note: They must complete an eligibility form. Requests for transportation must be made 48 hours in advance of appointment.).
